Under "US VISA APPLICATION DOCUMENTS PICK UP/DROP OFF" Click on "click here"

Make selection, scroll down, click on the "DhL pickup...." Click on book appointment, enter your details select a date two days ahead otherwise you will pay....just use your intuition from there, they will generate a pdf for you.
