REQUIREMENT FOR PRODUCTION INSPECTION OF PREMISES FOR REGULATED FOOD, WATER AND COSMETICS
Please arrange four (4) photocopies of the documents in the order it appears below and present originals for sighting.
1. Labeling
The labels of the packaging materials should be in the following pattern.
- Name of product/pack size (Top Portion)
- Batch No.
- Manufacturing Date:
- Best Before:
- NAFDAC REG. NO.
- Factory Location Address (Not P.O. box) (Bottom left Portion)
2. Evidence of Payment
3. Certificate of Incorporation of Company
4. Evidence of Trade Mark Registration
5. Organogram of the Company with Names and Qualification of the key officers (e.g. Managing Director, Production Manager, Quality Control Manager).
6. Letter of Appointment and Acceptance of key officer
7. Credential of the key officers.
8. SOP for production
9. SOP for quality control
10. SOP for cleaning of factory premises and equipments.
11. SOP recall and Distribution channels.
12. List of production Facilities qualify Control
13. Batch formulation (Not Applicable for packages Water).
14. Retainership Agreement with a Hospital or Clinic (with names and signature of both parties).
15. Food Handlers Test/Medical Fitness Tests which should include.
- Sputum test
- Urine test
- Stool test
- Chest X-ray
- Widal test

16. Analysis of raw Materials and Finished products.
17. Agreement with Fumigation Company for regular fumigation of the factory with name and signature of both parties
18. Current fumigation Certificate of Factory. (This should by done quarterly).
19. Geological Survey Report of borehole
3. If successful inspection is conducted, an acknowledgement letter of recognition as a producer is issued. This enables the producer to go ahead and get the product registered.
4. Purchase of registration form with a Bank Draft of Two Hundred and registration and Regulatory Affairs) Central Laboratory complex, NAFDAC, Oshodi, Lagos state with the following documents.
i. certificate of Analysis.
ii. Certificate of Business incorporation.
iii. evident of trademark registration
iv. Reception of all payments.
v. 3 well labeled samples for veiling.
vi. Acknowledgement Letter for sample Collection.
5. Upon release of satisfactory laboratory result, product is presented for approval.
6. After approval of product, applicant is communicated to pay the prescribed fee).
7. (a) PAYMENTS (Note: All payments should include 5% VAT of the prescribed fee).
