1. Acceptance letter.
2. Guarantor's letter.
3. Affidavit of Non-membership of secret cult.
4. Affidavit of good conduct.
5. Certificate of Local government of origin.
6. Birth certificate or affidavit.
7. NYSC certificate
8. B.Sc certificate.
9. Msc certificate
10. Neco or Waec certificate
11. Uniben Admission letter.
12. Referee form
13. Brown a4 envelope
14. Transcript.
15. Clearance slip
16. Acceptance fee payment print out
17. One passport
18. Prospectus receipt (which will be given to you after paying 3500 naira)

Go to any high court, tell them that you want affidavit of good conduct and affidavit of non membership of cult. They have the format there already
