It's form 2B and 2C, to be completed by you and also by your employer. Then I think there is a third form that's much simpler, and is to be submitted along with NYSC ID card on POP day (that's the one I think is the terminal letter, showing you've finished service so you get the discharge certificate).

We've being asked to come pick our terminal letter. POP definitely loading... 50%
