One and same person affidavit will be fine. He should use his names as it appears on his passport for IELTS exam, and for the rest of the application for EE.

Good morning all,

I have a concern. My hubby who is the PA used his middlename as his surname in his passport. All our kids and I are bearing his middlename. His documents including ECA all have his father's name. He is about take IELTS. Please what should we do?
So instead of Mohammed Akinsola Onyeka (all documents except IELTS which may come out different), he used Onyeka Akinsola. The rest of us are Onyeka Trump, Onyeka Buhari.
