Reference letter is not mentioned in TRA application guideline, however it is good to have as i think it might be requested down the road.

Completed employer template is the form you are expected to give your employer to be filled with details
For each year means if you you changed jobs within the period (yrs) of claim you are expected to give the employers seperate forms
self employment : form to be filled by the self employed.
