Affidavit is a valid legal document, it's even accepted outside the country. I have using mine for over 9 years, it's accepted anywhere.

YES, very important!, as well as a birth certificate.

I had mine on the 8th, it was very fine....it was more of document screening, only one question was asked as related to my course of study.

For those asking about birth certificate, i did mine today using a sworn affidavit of age declaration and it was accepted. She didn't even mention birth certificate at all. probably she was in a good mood but my affidavit was accepted.
