According to the muslim , the mad people will be judged based on their life affairs when they were in sane ( I mean their period of good sanity). If it is external inflicted madness, The mad people sins will be transferred to the person who inflicted them with madness in their life time, if it is self inflicted madness ( like drug related ) , then they will be hold accountable for their sins . And for the those born mad, that is Allah will.

We are not the ones who created them but however we know that madness is curse and cursed person is proof that he is guilty.

People are not generally born mad even the baby born by a mad woman is not mad.

Interesting question. In my opinion, the state they were before the madness is what will matter. If the madness was inflicted on them by someone else, then the state of their affairs with God before they lost their mind will be the factor to determine their destination, hell or heaven. If however the madness was of their own making, e.g drugs or involving in some diabolic or occult activities which affected their minds, then they have received on earth the punishment for their action and in the hereafter, hell.

Someone asked what if the person was born mad. Well, I have never heard of a case of someone born mad, but if that were possible, then I think the fate of such a person will be the same with that of a baby born dead, or who died at infancy before the age of accountability.

Many of them are not evil, it's all about perception of reality, if your reality is God alone you'd operate from a completely different realm and everyone around you would start from calling you "strange." On a personal level, some folks may not be able to balance the realities of the realms from which they operate with the material or physical realm, they're taken to hospitals by their family accusing them with big grammar, schixophre, miopionia and so on, fix them on drugs and make their matter worse. In light years, you can still never understand the art of acting which comes in various forms.
