Affidavit that's you're the same person, don't bother with that newspaper announcement thing. Affidavit from court and you're good to go.

it depends on your agency. Just put your benchmark to be on the sixth month coz that's the max. Or you can state the agency so that people with firsthand knowledge on the particular agency tell you
