I think the best option should be extraction so that whatever is eating that tooth doesn't crawl to the others. I'm Not a medical personnel though

Take him to a dentist. Depending on the level of decay from their assessment, filling is possible. If not, then it has to be extracted. You don't want to experience the pain of toothache trust me.

Its best you pull it out and maybe put in a replacement tooth. I once had a hole similar to this and it had to be pulled out, the hole was too wide. that tooth will likely break if they tried to clean it and fill.
