Basic123:If you are making on your own clinical diagnosis by only the observation of sign and symptoms, nobody is quarrelling with you. You are well within your right as a physician. Whether you patients would be satisfied is a different thing entirely. However, headache, Fever, Anorexia are symptoms shared by thousands of different diseases, the Medical Laboratory is there to make differential and definitive diagnosis based on the patient's demographics, clinical manifestations etc..as it did during the COVID-19 global scourge. Clinical Laboratory Diagnostics is entirely within his professional purview. As long the diagnosis involves medical laboratory input, he is requested to write a report based on his investigatins. He is an expert,expected to be very much grounded in one of the many subfields of medical laboratory diagnostics. The job descriptions are clear, the MLS is well-versed and can discuss the diseases he investigates in the lab. Infact he sees the causative organisms and agents of these disease more often than you do as part of the exigencies of his job. So, pray tell, why do you think his opinion should hold no water when diseases and diagnostics are being discussed? |

DNAPro:Who is saying that his opinion doesn't hold water when he actually help the doctors in SOME of the diagnosis...and you are not the only ones that do,Nurses also help with their vital signs and fluid charts.Although,there are thousands of diseases whose diagnosis are purely clinical.No test is needed to confirm,especially most psychiatric illness and some surgical diseases. The point is that he doesn't have the final say and he doesn't have any reason to argue.Give him your lab result,you have done your job.The doctor will combine the result with so many other information he has that the scientist doesn't have to make his conclusion.He will also never interfer in the way you mix your chemicals or view your microscope to get your result even though he has an idea. There are times doctors disregard lab findings base on so many other parameters and guildelines.Its not as straightforward as you think.For example,that a nurse gives a doctor high BP reading doesn't mean he should start treating the patient for hypertension. There are so many things to be considered,including some of the informations that might come from your lab to make that decision. That your second to the last paragraph is not your work,although you may have an idea just like the pharmacist and other paramedics do.Just like the doctors may have an idea of some test procedure,but it's still not his work! Doctors respect you and appreciate the good work you do to make the whole management go smoothly. Everyone should just maintain his lane. God bless us |

Do you carry microscopes during ward rounds? If you dispute it, you can provide counter evidential facts. We are on a public forum, and people are reading and learning. Basic123:Do you have any data on the prevalence of clinical misdiagnosis in our hospitals? It was 32.4% when I did my M.Sc few years ago. Are we sure the disregard of medical test results do not contribute to these figures? Nigeria is one of the easiest countries where you can get away with medical malpractice/negligence. We all know that and we know why. Illiteracy on the part of the patient/patient's relatives contributes to a part of it. I am very much aware that many patients and relatives are now getting to know their rights in a country like Nigeria with a chaotic healthcare system. May God help us all. |

DNAPro:Alot of factors can contribute to misdiagnosis,not only disregard of test result(which most of the time is valid,study medicine and you will know why). Laboratory errors from your end can also contribute,doctors poor clinical decision making,Nurses ineptitude etc.That is why the doctor whose the bucks end on his table have to be deligent and have high index of suspicion when your lab result,Nurses vital signs and radiographers results and not in keeping with his clinical assessment.There was a reputable hospital where I had once worked whose malaria test was never negative after working there for good 6mths.I had to alert the CMD and lab head. So,disregarding results (A perfect example is when you grow staph aureus in ?CAUTI) is the least cause of misdiagnosis compared to what I listed above. |

Basic123:We are also well aware that Hospitals and Physicians employ only Lab Technicians or graduates of Science Lab Tech instead of licensed Medical Laboratory Scientist to run their labs in order to cut costs. Don't you think the employment of quacks and non-qualified personnel contribute significantly to these medical errors and disregard of lab tests? |

Basic123:You don’t try explaining these things. 🤫 At the end of the day, 10 to 20 years from now, they will be all over their HODs begging for admission slot into medicine and surgery for their children. Been there, seen all those. Allow them bask in their grandiosity of making diagnosis while increasing resistance of organisms to drugs, as well as increasing the incidence of renal failures with their diagnostic acumen. Them choke for Aba. No worry yourself. Na OSCE trained doctors dey worsen the matters sef. Every small complaint, lab test and US. No proper Hx taking and physical examination. |

I put it to you that BIOCHEMISTRY is the hardest course to study nd when you talk about stress that is true. We have a course named intermediate metabolism in biochemistry, just this course contains several topics wich are so hard.
