May her soul rest in peace. Upon investigation, I won't be shocked if the scaffold is made of these substandard steels all in a bid to cut costs. At least, if you cannot afford strong and heavy load bearing steel the landlord or whoever in charge should have considered concrete slab.

Most causes are due to the re-enfoce steel, they tend to carry more load than what it's actually designed to carry.
Another cause is the "height"(which many pple don't take into consideration) of the overhead tank, without enough re enforcement to support the load, over time the steel will collapse. That is what cause the incident in our case.
Lastly, substandard materials.
In my own opinion, water tank space should be included in the roof of the building when planning the design of your building, that is a better place where it can sit.

The positioning or location of water tanks may not really be the cause but lack of solid structure to carry them.
