Likely sweat in this case. And maybe any skin he may have shed while he sat. But truth be told, that paranoia is likely the reason Kim is alive today. Those who refused to be paranoid are dead. Israel famously killed Nasrallah days after Nasrallah had agreed to a ceasefire. They did the same to Iran too, and tried killing their chief negotiator.

Just some days ago, it was the Yemen political leaders getting wiped out because they believed Israel would never attack civilian cabinet members who were not directly involved in the war. Meanwhile, their leader, Al-Houthi knows very well to not “lose guard” himself in public. Saudi has tried killing him several times, so he doesn’t put himself out there.

As for Kim, the North Korea-South Korea war is still ongoing. It ended in a truce and there was no peace agreement. So, Kim knows very well that the Americans & South Koreans can attack anytime.

Besides, anyone who is even conversant with the CIA knows very well the length they will go to get whatever they want. These are the same dudes who faked an immunization campaign just to get Bin Laden family's DNA. They also redirected the pipes in the toilets of Soviet presidents visiting the US just to grab samples of their poop. In a world with these type of people, you can never be too careful.
