Pharmacy screening is tomorrow ... Guys please that screening form ...is it the first and second page we are going to submit ... And I have one original of the full 3pages the rest are photocopies ... Is that right??

nope.. In the screening form,they don't even look at the school you attended at all.. All they are interested in is no 1-8 which includes ur name,course,score,reg no,state&LGA,and date of birth. So,u are good to go... Fire on..

yea dude. Submit the one with ur picture and school info(primary,secondary or both).. And the one which is meant to be signed.. Ignore the one with ur waec/neco grades on it..

What of recent affidavit of Name. To back up inclusion of name in any of the necessary documents dats not in d birth cert.
