Low Amniotic Fluid Levels: Oligohydramnios: Causes, Risks And Treatment

The amniotic fluid is the fluid that surround be baby while in the womb.

It protects your baby and aids in the development of muscles, limbs, lungs and digestive system.

Amniotic fluid production starts soon after the amniotic sac forms at about 12 days after conception.

Initially amniotic fluid is made up of water that is provided by the mother, and from around 20 weeks gestation the urine produced by the baby becomes the most significant component of the amniotic fluid.

The amniotic fluid provides an environment for the growing baby to move and tumble while in the womb.

In the second trimester (#atgtrimester) the baby will begin to breathe and swallow the amniotic fluid, this is very paramount for the development of the lungs and kidneys.

In some cases the amniotic fluid may measure too low or too high.

If the measurement of amniotic fluid is too low it is called oligohydramnios.

If the measurement of amniotic fluid is too high it is called polyhydramnios.

What is oligohydramnios? #atgoligohydramnios

Oligohydramnios is the condition where the amniotic fluid is too low or little.

Doctors can measure the amount of fluid through a few different methods when you attend your antenatal care visit, and it is essential that this is done during your scans. (#atgscan1 #atgscan: more to come)

There are 2 common methods of amniotic fluid evaluation:
1. amniotic fluid index (AFI) evaluation
2. deep pocket measurements

*If an AFI shows a fluid level of less than 5 centimeter;

*Or the absence of a fluid pocket 2-3 cm in depth, a diagnosis of oligohydramnios would be suspected.

About 8% of pregnant women can have low levels of amniotic fluid, with about 4% being diagnosed with oligohydramnios.

It can occur at any time during pregnancy, but it is most common during the last trimester.

If a woman is past her due date by two weeks or more, she may be at risk for low amniotic fluid levels since fluids can decrease by half once she reaches 42 weeks gestation.

What causes low amniotic fluid?

1. Any developmental Birth defects in the kidneys or urinary tract of the unborn baby which could cause little urine production, leading to low levels of amniotic fluid.

2. Placental problems – If the placenta is not providing enough blood and nutrients to the baby, then the baby my not be getting appropriate nourishment and therefore stop recycling fluid.

3.Leaking or rupture of membranes –If there is a tear in the membrane that surrounds the baby : Premature rupture of membranes (PROM), could also result in low amniotic fluid levels.

This could present as gush of fluid or a slow constant trickle of fluid.

4. Post Date Pregnancy– A post date pregnancy (one that goes over 42 weeks) can have low levels of amniotic fluid, which could be a result of declining placental function.

5. Maternal Complications– Factors such as maternal dehydration, hypertension, preeclampsia, diabetes, and chronic hypoxia can have an effect on amniotic fluid levels.

What are the risks of having low amniotic fluid?
The risks associated with oligohydramnios often depend on the gestation of the pregnancy.

The amniotic fluid is essential for the development of muscles, limbs, and most especially the lungs, and the digestive system.

In the second trimester, the baby begins to breathe and swallow the fluid to help their lungs grow and mature.

The amniotic fluid also helps the baby develop muscles and limbs by providing plenty of room to move around.
If oligohydramnios is detected in the first half of pregnancy, the complications can be more serious and include:
* Compression of fetal organs resulting in birth defects
* Increased chance of miscarriage or stillbirth
If oligohydramnios is detected in the second half of pregnancy, complications can include:
* Intrauterine Growth Restriction (IUGR)
* Preterm birth
* Labor complications such as cord compression, meconium stained fluid and cesarean delivery

What treatments are available if I am experiencing low amniotic fluid?
The treatment for low levels of amniotic fluid is based on gestational age.

If you are not full term yet, your doctor will monitor you and your levels very closely.

If you are close to full term, then delivery is usually what most doctors recommend in situations of low amniotic fluid levels.

Other treatments that may be used include:

1. Maternal re-hydration with oral fluids or IV fluids has shown to help increase amniotic fluid levels.
Mother treatment methods are not readily available in most centers includes : Amnio-infusion

I find it hard to sleep inside a mosquito net, I always feel as if I'm choking whenever I'm sleeping with it and when I wake up I always v burning skin. I spread it outside for one week self before using it. Is there any other alternative apart from mosquito nets for prevention of malaria, my dd don't like sleeping inside her cot again, she prefers sleeping beside me. As I'm even typing this my face is on fire, I just removed the mosquito net and spread it outside again.
Sorry for the long story but please help a sister.

The mosquito net you have is the chemically treated one. When i use it, my whole body itches me and it feels as if they poured pepper in my eyes. So i switched to the untreated one. You can also try washing it to see if it'll calm down.
The treated one is supposed to be the best but some people's skin cant stand it.

I also react to the treated mosquito net, my whole body feels burnt and I feel stuffy in it. I wash with detergent and rinse before use. Though I have stopped using it
