HEPATITIS

This is the inflammation of the liver which leads to liver injury and damage

Viral Hepatitis is the inflammation of the liver by infectious viruses like HAV, HBV and HCV

Viral hepatitis, accounts for more than 50% of cases of acute hepatitis.

Hepatitis A Virus causes only acute hepatitis and does not progress to chronic hepatitis.

Hepatitis B & C causes both acute and chronic hepatitis

Patients who are chronically infected may go on to develop cirrhosis and hepatocellular carcinoma (HCC).

Hepatitis B & C Causes direct damage to the hepatocytes => Cirrhosis of the liver => Hepatocellular Carcinoma (HCC) => Fulminant Hepatic Failure (FHF)

Chronic hepatitis carriers remain infectious and may transmit the disease for many years

The pathogenic hepatitis viruses includes HAV, HBV, HCV, HDV and HEV.

Whether HGV are pathogenic to humans remains unclear.

CAUSES OF HEPATITIS

Has various causes:

Infectious causes

Viral (HAV from consumption of contaminated food and water), HBV, HCV, HDV and HEV)

Bacterial

Fungal

Parasitic organisms

Noninfectious causes:

Alcohol

Drugs

Autoimmune disease

Metabolic diseases.

HAV, HBV, and HCV are responsible for more than 90% of cases of acute viral hepatitis.

HAV is the most common cause of acute hepatitis,

HCV is the most common cause of chronic hepatitis.

MODE OF TRANSMISSION

Fecal-oral transmission frequency:

HAV (+++)

HEV (+++)

Parenteral transmission frequency: Through needle pricks or blood transfusions or shared needles by I.V Drug users

HBV (+++)

HCV (+++)

HDV (++)

HGV (++)

HAV (+)

Sexual transmission frequency:

HBV (+++)

It is readily detected in serum, semen, vaginal mucus, saliva, and tears.

The virus is not detected in urine, stool, or sweat.

It is transmitted more easily than human immunodeficiency virus (HIV) or HCV.

Infection is associated with vaginal intercourse, genito-rectal intercourse, and oral-genital intercourse.

An estimated 30% of sexual partners of patients infected with HBV also contract HBV infection.

HBV cannot be transmitted through hugging, or household contact (e.g. sharing towels, eating utensils, or food). Sexual activity is estimated to account for as many as 50% of HBV cases

HDV (++) requires the presence of HBV to replicate. The sharing of contaminated needles in IV drug users is thought to be the most common means of transmitting HDV

HCV (+) transmitted via parenteral (needle prick and blood transfusions) and sexual intercourse.

HAV (+) through sexual contact (anal-oral) in Gay. The virus spreads from person to person via the fecal-oral route. Contaminated water and food, including shellfish collected from sewage-contaminated water.

Perinatal transmission frequency:

HBV (+++)

HCV (+)

HDV (+)

Sporadic (unknown) transmission frequency:

HBV (+)

HCV (+)

HEV is the primary cause of enter ally transmitted None None (NANB) hepatitis. It is transmitted via the fecal-oral route.

It can also be transmitted vertically to the babies of HEV-infected mothers.

It is associated with a high neonatal mortality

SIGNS AND SYMPTOMS OF HEPATITIS

Jaundice

Nausea

Vomiting

Anorexia

Abdominal pain

Fatigue

Malaise

Increased Alanine Aminotransferase (ALT) values (>1000 U/L)

Hyperbilirubinaemia

COMPLICATIONS

Severe acute hepatitis may lead to:

Acute liver failure

Fulminant hepatic failure (FHF) (acute liver failure + hepatic encephalopathy)

Chronic hepatitis may lead to:

Liver Cirrhosis

Hepatocellular Carcinoma (HCC)

End stage liver disease => Death

PREVENTION

For HAV

Personal hygiene

Avoid patronizing food vendors

Avoid Oro- Anal sex

For HBV

Avoid needle pricks for health workers

Avoid sharing of needles, toothbrush and razor

If you are single total abstinence from sexual intercourse

If married stick to your partner

Avoid multiple sexual partners

Avoid indiscriminate MouthAction

For HCV AND HDV

Just like HBV

For HEV

Just like HAV

ALL YOU NEED TO KNOW ABOUT HEPATITIS

A: Hepatitis is an inflammation of the liver. The condition can be self-limiting or can progress to fibrosis (scarring), cirrhosis or liver cancer. Hepatitis viruses are the most common cause of hepatitis in the world but other infections, toxic substances (e.g. alcohol, certain drugs), and autoimmune diseases can also cause hepatitis.

There are 5 main hepatitis viruses, referred to as types A, B, C, D and E. These 5 types are of greatest concern because of the burden of illness and death they cause and the potential for outbreaks and epidemic spread. In particular, types B and C lead to chronic disease in hundreds of millions of people and, together, are the most common cause of liver cirrhosis and cancer.

Hepatitis A and E are typically caused by ingestion of contaminated food or water. Hepatitis B, C and D usually occur as a result of parenteral contact with infected body fluids. Common modes of transmission for these viruses include receipt of contaminated blood or blood products, invasive medical procedures using contaminated equipment and for hepatitis B transmission from mother to baby at birth, from family member to child, and also by sexual contact.

Acute infection may occur with limited or no symptoms, or may include symptoms such as jaundice (yellowing of the skin and eyes), dark urine, extreme fatigue, nausea, vomiting and abdominal pain.

Q: What are the different hepatitis viruses?

A: Scientists have identified 5 unique hepatitis viruses, identified by the letters A, B, C, D, and E. While all cause liver disease, they vary in important ways.

Hepatitis A virus (HAV) is present in the faeces of infected persons and is most often transmitted through consumption of contaminated water or food. Certain sex practices can also spread HAV. Infections are in many cases mild, with most people making a full recovery and remaining immune from further HAV infections. However, HAV infections can also be severe and life threatening. Most people in areas of the world with poor sanitation have been infected with this virus. Safe and effective vaccines are available to prevent HAV.

Hepatitis B virus (HBV) is transmitted through exposure to infective blood, Fluid, and other body fluids. HBV can be transmitted from infected mothers to infants at the time of birth or from family member to infant in early childhood. Transmission may also occur through transfusions of HBV-contaminated blood and blood products, contaminated injections during medical procedures, and through injection drug use. HBV also poses a risk to healthcare workers who sustain accidental needle stick injuries while caring for infected-HBV patients. Safe and effective vaccines are available to prevent HBV.

Hepatitis C virus (HCV) is mostly transmitted through exposure to infective blood. This may happen through transfusions of HCV-contaminated blood and blood products, contaminated injections during medical procedures, and through injection drug use. Sexual transmission is also possible, but is much less common. There is no vaccine for HCV.

Hepatitis D virus (HDV) infections occur only in those who are infected with HBV. The dual infection of HDV and HBV can result in a more serious disease and worse outcome. Hepatitis B vaccines provide protection from HDV infection.

Hepatitis E virus (HEV) is mostly transmitted through consumption of contaminated water or food. HEV is a common cause of hepatitis outbreaks in developing parts of the world and is increasingly recognized as an important cause of disease in developed countries. Safe and effective vaccines to prevent HEV infection have been developed but are not widely available.

Bottom line is; Hepatitis is more like a rare liver disease caused by a parasitc organism which attacks the liver. This alone makes it very deadly

Trinity healthcare is determined to care for those living with Hepatitis B&C. if you are on this thread, the following information may be very important to you or your love ones. If you are living with hepatitis B, you have to ensure that all those living with you are vaccinated. if you are positive to the virus and you are pregnant, be sure to administer Hepatitis B Immunoglobulin to your baby on the day of birth, this will serve as a post exposure prophylaxis to prevent the newborn from contacting the virus from the mother. After weaning the baby, you can immediately commence a treatment plan for Hepatitis B. Here, a specific antiviral drug like Hepantivir may be very useful. Be careful to monitor the newborn for HbSAg. If you are not infected with Hepatitis virus, please hurry and get vaccinated, it is the only insurance policy against the virus. In addition, avoid any activity that will lead to exchange of body fluids including blood, semen, sweat with others. Essentially, the prevention of Hepatitis B is key to an Hepatitis-free society. For further enquiries, contact Trinity Health care on 09031652000, 08028791004 or send an email to healthcaretrinity1@gmail.com.
