it's 20year old, why? because 20 and year are adjectives adding more meaning to the noun old. one of the rules is no plural for adjectives. that's y we have sth like
3 man panel or 3 man defence, and not 3 men panel or 3 men defence, hope have clarified.
I rest my case.

So it depends on how you apply it.

Op, it depends on the usage.
When it's used as a adjectival phrase. 20-year-old is correct. But when it's a verb construct. It becomes '20 years old'.
E.g.
The 20-year-old football star has just been signed by Arsenal football club for a club record fee.

The football star who has just been signed by arsenal football club is 20 years old.


Note: in the first scenario, it's used as an adjective of 'football star'. While in the second scenario, it's used as a verb construct to the verb 'is'.

This post I quoted should be showered with likes! I mean one can't explain any better. "20 year old girl" is used sometimes when the name of the girl in question is unknown so it qualifies the noun girl.

smiley I think when you are reporting or referring you use 'a 20 year old lady', but if it's quite personal you can say 'i am 20 years old'. You can see that that there was 'lady' in the first one to show reference but u don't need it in d second one cos its obvious to the hearer what gender u are or so.

This appears to be the most correct answer to the question.

On wether the addition of hyphen is compulsory, YES it is. When you say "A" the next object which the "a" describe must be SINGULAR. The hyphens makes it (a 20-year-old) singular.
