You don't need to fill the IMM5257, this form is meant for visitors visa applicant.

Hi guys! My study permit was approved for May entry!!!

submitted my application on 1 March (paper route)
Got Medical request on 26th April
Passed medicals on 11 May
Got approved on 22 May

Submitted the following documents:

MSc Admission letter (MUN with full funding)
All CIC forms
3-page letter of explanation.
1-page description of the funds available for my stay, where I gave a breakdown of how much goes into, tuition, rent, feeding etc for 1 year
Bank statement of my sponsor (uncle) and his letter of support
Bsc transcript
Birth certificate
Police criminal record certificate
National service certificate

NB: I am a Ghanaian applicant

This Canada process is so unpredictable, no update since I last did my medical. It almost 3weeks. I passed my medical was thinking by now I should be updated about the final decision..

Is your profile still showing background check in progress or its completed? For how long did it show eligibility review before your medical call?

I said it previously, online applicants are meant to fill two forms namely:
IMM1294E and IMM5645E.
You're not expected to fill the IMM5257B (It's for visitors visa applicant)

I can't really tell. it was already under eligibility review when i linked my application online (thus after one month of submission). It remained under review until the final decision was made.

If the names are spelt wrongly from the name on your passport, you should go and rectify the names on your certificate and NYSC. Otherwise, go and do your passport in the name on your certificate and NYSC. You dont want CIC thinking you burrowed someone’s certificate with almost similar names like yours.
i can't change the name of my passport na, every other document is with the same name only the NYSC and Bsc certificate. Would try and reach my school to make to know the steps I would take if it wouldn't take much of my time I would ask for a reprint. By the way the school I am applying for doesn't require my Bsc or Nysc qualification....Its just a training school for coders and the requirement is just IELTS a skype interview to see if I am passionate about coding and possibly pass codes I have worked on, do I still need to attach BSC and NYSC certificates?

The box on the IMM5645 for family members is not sufficient. I have 10 siblings..lol. The box provided covers 7. I know this information is very important for future reasons, so i will like to provide all the ncames. Is there anyway i can go about this?

So you haven’t even obtained admissions? If so, go correct all your errors on form now. If your transcripts bears your correct name, in fact that will supersede certificate and NYSC. But you need to correct all error now. Best wishes!

Not necessarily. I also used 5645 in my online application. I think its fine if he goes ahead and uses what he has been required to submit in his application.

Print an additional one and fill the remaining names in the relevant fields. Then you merge the two and upload. I did the same in my application, two mothers.

Yes, ignore it. As you can see, it is an optional document. There will be no need to fill that form. Once your study permit is approved, a temporary visa will also be issued without a separate application.

Yeah Attorney jnyuwa02, sounded as if a sworn notarized affidavit explaining the errors in the applicant name won't work and he further said that the visa officer will think the applicant is making use of someone else's certificate just to acquire visa. Which is wrong and I seize not to agree from his perspective.
I want to also believe that Attorney jnyuwa02 hasn't come across an applicant that used a sworn notarized affidavit and got approved without visiting the issuing institution (body) to effect a change in name. That's why he is so doubtful about it. Well, I have seen people present affidavits and it works good!
You have made a valid point. A sworn affidavit would suffice at any instance for discrepancies in documents. It is a legal tender and importantly used as an evidence to a claim. That fellow as you advised is fine with an affidavit and has nothing to worry about.
