No it is not a must.
Just put Nill on the middle name and make sure you fill other necessary info

You are to sign the Referee form,attached all the listed documents (Guarantors Passport and photocopy of ID card).They will shortlist applicants, not all applicants will be invited for screening and/or Aptitude test. Originals and photocopy of all your academic and professional certificates will be asked.

Nooo...you keep them and wait until when you are shortlisted. you go to screening/test/interview venue with it
