You have to attach the passport manually on the for after you print it... It will be signed by the person who will fill your attestation form.

Just wait a little and try again, its from their end I guess..It happened to me too but It went well after a while

Really... Well last year NIN was not required for registration and so it was no needed. Besides i think the slip should be ready in less than a month or two atleast thats what i know for Lagos centers well if you have the first print out and you know your NIN, you should register and wait to be shortlisted for zonal screening and if you are go with and pray God favour you. Dont forget to explain your situation politely to the officer when need be.

a letter headed paper would be a better option but as long as the senders address, name, signature and stamp is there i think that should do it.
