how did u achieve this set up??

I got most of the icon images from twitter and Googled some that the person who shared didn't have and downloaded.
I used two apps, X icon changer app and simple photo widget for changing the icons and putting photos as widgets in your home screen respectively.

X icon changer I downloaded was a modded one cos the free version wouldn't let you change icon images with photos if you don't buy premium.
