If you have online banking app. Download and print out your bank statement from there. It normally comes with stamp, example gtb bank.

But for the other questions, I advice you contact mba or visit their office.. But I don't think it will affect your Roi, the only issue is that u may not be able to withdraw it if you don't submit all the required information
