Is it possible for an application to be updated as refused after PPR and passport still being stamped upon delivery?

Yes it is possible, in my own case i got a refusal after 3 months of waiting. The reason was because i failed to go for the medical exam due to some personal issues. That's what in most cases delays the stamping of passport. So i advice that if they requested for medical you go for it and upload the e-medical slip you'll be given after the medical test or If you've already done an upfront, you can also upload the e-medical slip you were given after the medical exam.
