There is no residency training in clinical pharmacology anywhere in the world!!!

The qualification for the position of clinical pharmacologist is a PhD in clinical pharmacology and the individual must not necessarily be a medical doctor. Even a BSc pharmacology graduate that has done a PhD in clinical pharmacology can hold that position. The role is to give a guideline and coordinate clinical trial of drugs.

Btw, there is no conflict between the role of a consultant pharmacist and that of a clinical pharmacologist.

You guys are stooping so low by dignifying all these mere drug dispensers with brilliant answers. See the questions that put them off balance. Leave all these dispensers alone. You guys are far ahead of them. Their problem is inferiority complex. They don't even know that pharmacology is just an integral part of the voluminous work of a doctor. Yet, they are still being put off balance in pharmacology. Pharmacists are as useless in these country as P in psychology. Chemists are doing better than some of them. The only thing they know how to do best is rubbing shoulders with doctors. Lab scientists are more important than them. They will now start competition for space and job with chemists.

Sentiment apart, Pharmacists are very useless in this country. I would rather encourage people who may not be interested medicine/Dentistry to consider medical lab science than encouraging them for pharmacy just because, unless the person plans to relocate to another country, they are valueless and useless. Diligent lab scientists in the house.... I 'm proud of you guys even though there are few rogues among you guys too. I hold you guys in high esteem for brilliant test results from you guys. Come to think of it, why would I encourage someone to go and spend 4or5 yrs in the University and at the end of day, he will now start competition with chemists. The lucky ones that get dispensing jobs in the government hospital will now be rubbing shoulder with doctors due to low self-esteem and inferiority complex. Imagining going to the ward to review a patient that you are not invited to come and review and at the end of the day whatever you do doesn't make any difference or affect the line of treatment. Yoruba call it '' more gbo, mo ya ''. Shame on the Nigerian Pharmacists. No doctors will go and review a patient unless he is invited.

The nurses too, will claim they are the owners of the patients and they can do whatever any doctor can do apart from surgery. I pity you guys. what they know how to do best is to put their big butts in a very tight white gown and be tempting doctors and the patients with there laps and butts and then start gossip in their cubicles while patients need their attention. Many auxiliary nurses are more skilful and with high sense of mannerisms than our big-butted self acclaimed skilled nurses. Let me stop here for now.

I'm not a toxicology pharmacist...I'm not even in the clinical environment and believe me if you should meet with one who's in the same area of specialty as you and though he wouldn't be doing everything like you do cos of job description and division of labour, there's nothing you know that he doesn't have the knowledge of and he'll match you bumper to bumper if not even better than you...Could that be the reason NMA is fighting specialisation in pharmacy practice? saying subtly that we should concentrate on drug discovery and production. The health field is an evolving one with lots of innovations coming out weekly...naija drs should try to evolve too. The way NMA fought pharm.D then was like these guys are going to be trained to do drs stuff. That shouldn't be your fears as we know our areas of expertise and job description and as I will always say you guys should accept the fact that the dispensers of yesterday are today's clinicians. Our scope of practice keeps on expanding by the day. In climes where things works even though the dr leads the team he alone cannot reach conclusion on clinical matters. it's not only medical care that gets the patient well, there're other types of care too. It's a joint and team effort not this thing in Nigeria where the dr is all knowing.

This wannabe thing is another thing i can't understand. who brainwashed you guys to think that everyone in health sciences wanted to study medicine? for God sake medicine was my second choice and if you want evidence i can upload it here. ideally i shouldn't have given a response to this but just to put everything in the right perspective i it's needful to give you an answer although I'm surprised that you're being taught this at postgraduate level in medical sciences since you're a resident dr.
I will answer your questions one after the other.
1. The commonest route of administration of NAC is the intravenous route because onset of action is fastest through this route. IV route is also preferred because patients with PCM toxicity are always unconscious. If there is some levels of consciousness activated charcoal should be administered too.
2. The usual time to act after pcm toxicity is between 8 to 10 hrs but even at that anytime such a patient present to the hospital he should still be given NAC so late presentation should never preclude it's administration.
3. The site of the liver mostly damaged is the centre lobular because its a highly perfused part. The metabolic enzymes for pcm metabolism are are also found in high concentration there too so the most affected part.
4. Type of necrosis is fulminating hepatic necrosis.
5. signs of toxicity usually occur in four phases. If you are a good you should know.
6. Those signs are majorly encephalopathy, cerebral edema, raised intracranial pressure and of course jaundice.
I can't answer all your questions now...i just got back from work. I'm hungry but shocked that this what you're being taught in your so called post graduate studies...If you meet with a postgraduate clinical pharmacist you'll be ashamed of yourself.
Next time ask questions that are evidence base not this academic one.
No matter how you try to present yourself from research to health care the pxcist is your Lord

As I walked into ARD secretariat yesterday, around 11am, I overheard someone asking another person, what are the symptoms of malaria in pregnancy?

With style, information from his colleague, who was not on uniform.

It then occurred to me, that this ignorant Nigerian doctor, who earns nothing less than 250k every 30 days from the federal government, must be among the hoards
of doctors presently attending to pregnant women at the antenatal clinic where all nurses and midwives are on strike!

This is why the rate of patients deaths at the hands of ignorant and materialistic (NMA and ARD) doctors in UBTH is very high.

After 6 years studying medicine and now, probably on residency, this Nigerian doctor does not know the symptoms of malaria in pregnancy!

What a disgrace!

No wonder they refer to themselves as 'DUCK A'

Everyday, you see many UBTH 'Duck A' carrying big foreign textbook up and down! They ride SUVs and move about with electronic gadgets.

They don't know anything on their own, except their salary and how to quickly open their private clinics. Some will even open google in your presence and be browsing for prescription to write for you, when you visit the hospital.

Ask many patients about the speed UBTH 'Duck As' will write prescription for you before you finish complaining about your health concern.

And if you want to know how proud and ignorant 'Duck As' in UBTH are, ask the pharmacists in the hospital who regularly cancel wrong prescriptions given to patients by dumb ass doctors.
If not for pharmacists and even nurses, the rate of patients deaths would higher than what obtains presently, prior to the strike.

Even before the strike, students doctors on residency are among those in charge of UBTH. All the wards and clinics, including GPC are colonized by students doctors. The consultants are hardly around. The vast majority of patients who need consultants services are attended to by students doctors of all categories.

That is why the 'Duck A' above, who obviously has not spoken with the consultants assigned to him for a long, had no choice but to ask his colleague about the symptoms of malaria in pregnancy.

So, please nairalanders, make una help me educate this Nigerian 'Duck A' for UBTH on this question:

Any information or advice will be highly appreciated.

So UBTH 'Duck A', I don't care what your name is: stay glued to nairaland for a few days as they teach you what you didn't learn in medical school for six years about malaria in pregnancy.

There was an accident of a group transiting to Auchi from Benin. They were rushed to our A/E at Irrua.
There were no deaths, but minor bruises and cuts. But some have mandibular distortions and other degrees of fractures that may need some days in hospital and scans to be done. But they are all stable.
We swung into action trying to call some of our nurses, and store officers and union members to go help out, we didn't really ask who they were initially, and we didn't want to argue while accident victims suffer. They are all there now helping with the treatment of the injured as it was very clear that the Doctors were helpless at a point. I have called the HOD of general stores to help with treatment items. The HOD of Pharmacy has been on ground helping with pharmaceuticals, the Nurses have arrived as their chairperson mandated and we have the situation under great control.
After putting the anxiety to rest, we decided to really find out who the victims were and found out they are cleaners in UBTH not JOHESU members as being erroneously circulated in social media against the striking unions. A call to SSA chair, UBTH confirmed they are cleaners in UBTH & all outsourced staff and belong to no union in UBTH.
Well, we are happy to relax our strike a bit for the injured and we are happy to prove that treatments are difficult without JOHESU members. We are also happy to prove that we have a largely humanitarian mindset.
That's the summary of what happened.

What type of necrosis does it cause ?

Gimme the prognostic signs of the toxicity ?

And can you tell me about the symptoms, signs and management of fulminant hepatic failure ?

The above questions are definitely beyond the scope of your curriculum.
