You can't graduate on time because of incessant government induced strikes ( industrial actions), when you finally graduate, you cant get space to do residency (a core training module) unless you know someone who knows someone. That's why they take USMLE and PLAB to escape this cage and when they get accepted in US, IRELAND, UK and the rest of Europe, including African Trinidad and Tobago, they perform excellently.
The shame is on your country, it's politicians and the rest of you people clapping for a country which is 55 years post independence but has the developmental state of a toddler.
