Men d story has chenged about d submission. D news now is dat u ve 2 submite at ur state of origin. Even medical report dat was not meansion is d first item they will request frm u. I was there at NIS office lagos command, and i was not allowed 2 submite 4 d seek dat i'm not frm lagos.

Read d guidline vry well, it wasn't stated as one of d document apart frm brth certifc8 n letter frm ur local chrman, so its not d most impotant thing.
