Because the femur and tibia which articulates to form the knee, are weight bearing bones. You can see from the pic. that it is not a BKS (bilateral knee surgery) so the earlier he starts proactive steps like weight loss, the better for his health. That way, he might be able to save the other leg from further deterioration. Anyway, TKR (total knee replacement) should not be much of a big deal in the environment he is.

Looking at the picture, that looks like an ACL surgery on the left knee. It could be from earlier tears in his knee cartilage that wasn't properly cared for or from athletic injury.
