RIP to the dead. The safety and Welfare regulators in Nigeria have to leave up to their tasks. Underground hydrocarbon tanks are confined spaces and must be properly prepared to be worked in. There are lots of inherent hazards in such activities which may include presence of toxic substances like the LEL gases, CO2, hydrogen sulphide excess O2, etc. Other confined space hazards in this case may include collapse of subsurface, engress of underground water, limited egress and ingress space, etc. This activities, being potentially hazardous should have been covered with work permit and other complimentary docs, a rescue plan in place, an emergency by stander, competent personel, etc. The workers are also supposed be trained before embarking on such activities. People should know that if one don't plan for accidents, sure they will plan for one. We should spend more time on awareness and trainings... |

Africa is still so backward. This is why the rest of the world has relegated Africa to the background and always see us as unserious people. Safety awareness and practice is as important as the air we breath in other parts of the world. What killed those guys was H2s gas (hydrogen sulphide) it kills faster than a bullet. That's why confined space entry should only be by professionals that is after area has been tested for H2s gas. But Fatgbems and others in Nigeria will always never want to spend money on professional labour. It's a sad situation in Nigeria. |

This is why it is good to resign from job where your boss or employer always force u to do d unthinkable or what u don't like without considering your safety. U don't need to have plan b 4 u resign. The establishment lack safety equipments. The manager suppose to be prosecuted. I believe the victims demanded 4 safety facilities b 4 doing d job, but their boss shouted at them to do it casually or threatened to sack them if they didn't. Their boss might even told them that others have been doing it without safety equipment |

Poor Safety orientation. No confined space entry permit i guess cos this wouldnt have happend. This is what happens when employers feel no need for a necessary safety induction for their workers before employment. |

sameniceguy: Actually the DPR regulates HSE in all Oil and Gas activities in Nigeria. ISPON is just simply a professional body. They are not regulators and should/will never be. Remember almost 100 percent of the HSE Engineers in DPR are members of ISPON. While the contributions of ISPON might be welcomed by DPR in improving their regulatory functions, the Oil and Gas Industry and all its general and technical activities and service are regulate by the DPR. The DPR consist of 8 Divisions with of them being the HSE Division. The DPR has robust guidelines on HSE Management/Administration and Engineering with duties of the employer and employee on many matters including guidelines on confined space entry. As a matter of fact one of such guidelines, the MOSR 1997, is currently being reviewed a updated. While we can say that awareness might not be there amongst owners and operators of many oil and gas facilities (especially in the down stream segment), there are instances where Nigerians who are trained and informed just feel the training is too much talk and "It Is God that protects" and the "I am fine in Jesus name/insha Allah" attitudes. Usually for all major accidents DPR has a reporting system with a form (form 41) and an inquest will definitely be carried out. During the inquest, the head of the panel in DPR is granted the powers of a Magistrate and a full ROOT, IMMEDIATE/CONTRIBUTORY and REMOTE Cause Analyses will be effectively carried out. Bluffly: Well, I understand your point. But you must know that even by law, the DPR generates revenue through licenses,fines,penalties,fees, etc. I does not remove the fact that a regulator exists and within the oil and gas industry in Nigeria it is chiefly the DPR who has full legal and administrative backing to regulate the industry both Upstream, Downstream, Gas, HSE, Engineering and Standards and Strategy and Planning activities in the oil and gas industry in Nigeria. In the course of my experience if not for DPR and its enforcement of its HSE Regulations,so many Oil and Gas workers would be dieing like flies. Especially in down stream Petrol Stations, Depots/Bulk Storage Facilities and LP Gas Retail Plants. In the course of DPR doing its job it crosses and pisses off so many companies and powerful individuals who in the past (and even till now to some extent) will call their "friends" in Government who will then want to intimidate the staff of the DPR through their bosses. It is when you know some of their staff you will realize how the ''Nigerian factor'' tries to interfere with their work as regulators and how they have over the years proven to be resilient and have been able to stand firm in the face of this interference from Big companies and big individuals. However, one fact is that they are still understaffed as we speak today. One way or another our general attitudes as Nigerians and the fact that there are no consequences for so many of our dissident actions affects everything and even in the enforcement of regulations as important as HSE Regulations. It boils down to the same corruption and abuse of powers by many Nigerians and business operators and even some officials of these Government Agencies. 2 Likes |

But these people don't know the critical HSE rules of working in a "CONFINED SPACE" - Ensure you always have a watchman standing outside the confined space - Ensure you always have a breathing apparatus inside the confined space (Oxygen levels are usually lower in the confined space) ANOTHER NEEDLESS DEATH IN THE WORKPLACE. 1 Like |

OMG, this death could have been avoided. In any work situation / environment, safety is key. Job Risk Assessment (JRA) needs to be done, where associated risks related to the job are identified and action plan to avoid or mitigate looming danger is put in place in case of eventually. Once these assessment is completed, a PTW - Permit To Work is issued. Work Conditions such as - Confined Space, Work at height , Body Mechanics etc should be given utmost attention during both routine and non routine tasks. RIP to the dead. |
