Back Pain- Orthopedics help please by sd6: 6:31pm On May 19, 2017
Hello orthopedics

I have been having this back pain for 2 years now. It started mainly 5years ago when I started having pain while sitting down. Then this sharp buttock pain while walking that moves from one side to the other. It comes and goes but has become more frequent. Then stiffness of the lower back especially after resting on bed. It's difficult to bend over. I have had mri which came out clean. X-ray said straightening of the lumber lordosis. Please I need your advice. I also have heel pain. Medications have been of very little benefit.

Go to a hospital where there is a physiotherapist. You need advise about your posture,back exercises and therapy.
You are suffering from what is called lumbar spondylosis with sciatica.

Thanks for your response, I had about 10 sessions with a physiotherapist in a private hospital and it didn't help. Been to various govt hospitals and couldn't get a definitive diagnosis.
Doctors are beginning to think its psychogenic but I'm in real pain.

IF THE MRI is recent you can take the disc to another consultant radiologist for a second opinion , or repeat if it's old to be sure you haven't progressed to more severe nerve compression

Thanks, mri was in 2015. Access to consultant radiologists for secondary assessment is a challenge. I once demanded to see one at a scan center and was told diplomatically, she was not disposed. At public health facilities consultant orthopedic surgeons rarely take time to assess patients while at private clinics, consultation is expensive considering the fact that if you have chronic pain, you'd have to make several visits before there could be a definite path to proper diagnosis. Another mri? Maybe but none of those I've met have shown proper understanding of the images.

U need a repeat of those images, you can't assume that the status as at 2015 is still the same In 2017

There are many types of back pain,the one you feeling Mechanical pain.It is the most common cause of lower back pain, mechanical pain (axial pain) is pain primarily from the muscles, ligaments, joints (facet joints, sacroiliac joints), or bones in and around the spine.
Meanwhile you could use muscle relaxants or opt for massge therapy for back pain

Thanks, the abroad option has always been there but as usual, financial constraint. Hopefully I will get a breakthrough here in Nigeria.

Hello house, thanks for this insightful post. Not sure if there's any specialist to help.

Lately about a month ago, I began to have this lower back pain at my left side body coupled around my waist. well, I googled and I found out the possible cause which could be either kidney infection or kidney Stone. I proceeded to my school clinic (undergraduate students) , where the doc requested for a urinalysis, blood test and one other test which I barely remember the term.
Afterwards, the results were all negative; no symptoms of kidney stone/infection. she however, proceeded to referring me to another Specialist clinic for Abdomino pelvic Scan; The result was also negative. The scan report was stated as, I quote

"The right kidney is normal in shape (RT- 10.5cm x 5.09cm x 7.10cm) while the left kidney is mildly enlarged (LT - 11.87cm x 6.84cm x 6.85cm). Both kidneys are however normal in shape, position, outline and echopattern with good cortico-medullary differentiation. The pelvi-calyceal systems are no dilated. No renal Cyst, mass or calculus is seen".

That was the report. Doctor assured me nothing is wrong and recommended that whenever I feel pain, I should take pain relief medicine.
Note- The diagnosis covers-
Liver
kidney
gall bladder
Git
Urinary bladder.
(which were all Negative Reports)

Afterwards, I decided to go for another urinanlysis elsewhere, in a laboratory Result interpreted by the doctor was still negative.

Puss cells - pale and amber
Blood- Negative
Bilirubin- negative
urobilinogen- normal
ketone- negative
glucose- negative
protein- negative
nitrite- negative
leucocytes- negative
pH -7.0
specific gravity- 1.010

That was my Urinanalysis result else where, outside the school clinic.

Well, to consolidate the negative confirmation of this report, I probably need a finally check up to reassure me that all is well. Aside from Abdominal pelvic diagnosis and Urinanalysis which I did, what else is recommended? Is there also a need for a routine check up like the aforementioned?

Please, doctors in the house should help. can't afford to gamble this. I need to be 100% sure. What's your suggestion(s) please?

You may want to check if you have SIJ dysfunction or maybe coccydynia...the pain may be from your sacro-iliac joint or your coccyx? if you check our website physiocraft you'll see more details about it. I'm not sure what state you're located but just check our website physiocraft and maybe that can help you. All the best.
