NTA and gaffes represent a cross section of the decay inherent in our statutory institutions.

I doubt any action was taken to learn from this, enhance quality assurance processes and forestall a reoccurrence.

Though this gaffe may be overlooked by many, but truly, there is something fundamentally wrong with us. embarassed
