The patient looks about 10? That's the only reason for this free counsel.

Put your 2 palms around his neck and feel for any other mass from the base of his skull down to his shoulder on both sides, down to where the neck bones protrude at the back, and then to his collar bones in the front. Look for small round masses. Notice if they move, painful, or warm as you do this.

Next, go to his back, and hold your 2 hands around his neck, with your fingers over the Adam apple. Ask him to swallow. Do you feel a mass go up and down as he swallows?
How about the cheek in front of his ears, under it, and behind, any small masses?
Has he lost weight?
Any thing unusual in his mouth?

If the answer to all these is No, then it maybe what is called a lipoma. Not really malignant, but can cause pressure effect and dis-figuration, and then possible ulceration with pain and bleeding later.

I recommend a surgeon who is comfortable operating on the scalp, because it bleeds like hell!
A wide local excision will ensure that it doesn't recur. Close the excision with 2-0 or 3-0 vicryl or moncryl sutures with easy removal in about 10 days.
Patient may or not need antibiotics, because the advantage of the bleeding nature of the scalp is that wounds heal faster! Give local pain control, and post op anti-inflammatory.

Take him to the hospital for his head to be examined.

When a 13 year old boy looks like a 9 or 10 year old, that's not good. It's called failure to thrive, and you must take him to a doctor. Thanks.
