I am filling poland visa application form and I got to the where I will fill travel medical insurance. After several attempts of inputting so many date format like

01/02/2019 - 28/02/2019,
01-02-2019 to 28-02-2019,
2019-02-01 to 2019-02-28,
2019/02/01 - 2019/02/28,
2019 February
February 2019.

It is showing incorrect field.

Please can anyone who has filled the form before write back the correct date format to enable me complete the form.
