A Brief Introduction To Home Visit Report Form


Every service provider and social workers across most health non-governmental organizations must learn and know how to fill Home visit report form. The Home visit report form is a tool used by nost LOPIN projects and NGO’s/CBO’s accross Nigeria and other African countries. It also includes areas with high prevalence of HIV/AIDS epidemic and vulnurable locations around the world.

What is Home Visit Report Form

The home visit report form is a tool used for documenting services and planned interventions carried out, and provided to caregivers and their Orphan and Vulnerable Children (OVC), in their respective homes. Reporting Home visits without actually visiting the household under your care is criminal, and compromises the ethical integrity of your documentation as a service provider.

As a service provider or community volunteer, you must adopt the attitude of properly documenting your home visit reports, using the home visit report form. Home visit reports are to be done directly at the homes of caregivers and orphans and vulnerable children (OVC). Home visits are conducted to carry out or provide counseling, among other services. That is why it is important to learn how to fill home visit report form properly.

What Happens After Using The Home Visit Report Form


What happens after using the home visit report form is that you have to go back to the service forms of each of the Orphans and vulnerable children (OVC), and caregiver in a particular household, ticking the services you just rendered/provided within that quarter of home visit. Also, you have to tick the service(s) provided to the caregiver/household head, during the home visit. This is done, using the household vulnerability service form.
At the end of all documentation, the Home visit report form for that quarter should be placed, filed and stapled along with the care plan updated for the previous quarter. Also, generate a new care plan for the next quarter. The Home visit report form is usually filed in the household/caregiver’s file or folder.

At What Intervals Should Home Visits Be Done
?

Knowing when and at what intervals home visits should be conducted is as important as knowing how to fill Home visit report form. However, the timeframe for each home visits and service provisions is usually quarterly. That is to say, that home visits are to be carried out within and after every 3 months.

Reasons Why Home Visit Report Is Important

Do you want to know why home visit report is important? Look at the following reasons outlined below.
You see sometimes, during monthly club activities, there may not be enough time to provide some services. For instance, imagine a situation you have so many OVC’s and caregivers in attendance, in a particular month’s club meeting, and there was supposed to be HTS during the meeting. If time is not managed properly, not everybody may be tested that day. However, during home visits, you can test those you could not attend to during the club meeting.
Some services like the psychosocial support or encouraging a PL caregiver may require a one on one thing. Most times, HIV infected caregivers/OVC do not feel comfortable attending club activities because of the fear of stigmatization or exposure of identity. A caregiver or OVC like this must be visited at home to provide care, support, and counseling.
The principle of documentation, which is that “anything not documented is not done”, is another reason why Home visit report is important.
