Normally, there is no pain in the testicles. If pain does occur, it will not only suffer a great deal of pain, but also affect reproductive function. As we all know, testicular gland is a unique organ of men, in the scrotum, left and right, mainly secrete male hormones, which play an important role in the development and maturation of male reproductive organs, the emergence of male secondary sexual characteristics and the formation of spermatozoa. 

The epididymis grows close to the testes, divided into three parts: the head, the body and the tail. The head is connected to the testes and the tail is connected to the vas deferens.Epididymis is not only the only way for sperm, but also the place where sperm develop and mature. Therefore, the epididymis is closely related to the testes, both of which are often complicated with infection, known as epididymitis, which can cause severe pain. 

Acute epididymal or orchitis often occurs on one side, initially characterized by rapid enlargement of the scrotum with pain, which radiates to the ipsilateral groin and lower abdomen, accompanied by digestive tract symptoms such as high fever, chills, nausea and vomiting. The course of chronic disease is generally longer, often caused by incomplete treatment of acute epididymitis. 

Chronic epididymitis is mostly caused long-turn ineffective treatment or no treatment of acute cases. Urinary tract infection along the vas deferens to the epididymis aggravate the condition. The operation of transurethral instruments, frequent catheterization and indwelling catheter after prostatectomy are all factors causing epididymitis. 

The most common pathogenic bacteria are Escherichia coli, followed by Proteus, Staphylococcus, Enterococcus and Pseudomonas aeruginosa. Neisseria gonorrhoeae and Chlamydia trachomatis can also cause acute epididymitis. Most of the pathogenic bacteria retrograde into the epididymis through the vas deferens. In addition, bacteria invading the epididymis can also be infected through lymphatic vessels or blood to cause epididymitis.
