It's quite deep my interpretation of the picture can be likened to the perilous obstacles of life and how to escape it... the water represents challenges of live , the various levels of the shelve represent the means to escape the varying challenges life throws at us and the books connotes knowledge.

...in summation the picture tells us the more knowledge we seek and acquire as in reaching for the higher level of the shelve the more we keep our head above water by overcomong the challenges of life. grin

This picture depicts to healthy people what people with lung diseases feels like when doing basic things.

That's grasping for air, that asthmatic feeling.

Google "lungenliga".

They are a Swiss based Lung association who made that picture and several similar ones just to pass awareness message on lung disease.
I had to Google Lungenliga.
Thanks for this.
This gives me a clear view on how a child with breathing problem feels while doing normal things kids do.

She is spending precious time arranging the books, which signifies her job. But this job isn't convenient as she finds it difficult even to breathe.

She is so fixated on doing that stuff, which is irrelevant right now, that she doesn't even look around.

She could just walk up to the door and open it thus relieving herself.

Most of us are killing ourselves trying to fix things. When we could just walk out and save ourselves the trouble. Nothing is worth killing yourself over
