Treatment of asymptomatic hyperuricemia is not necessary, even though the plasma urate concentration may exceed 15 mg/dL (893 µmol/L) with diuretic therapy (particularly in patients with reduced renal perfusion due to underlying advanced heart failure). These patients are not at risk of uric acid precipitation in the tubules (uric acid nephropathy), since the elevation in the plasma urate level is due to an initial decrease in the rate of urate excretion. Gouty arthritis is also not common in this setting, occurring primarily in patients with a personal or family history of the disease. Prophylactic therapy with a potentially toxic drug (such as allopurinol) is not warranted to prevent a benign complication.
