In recent years, both nationally and globally, the use of questionnaires as a means of data collection in health-care research has increased. The growing focus on evidence-based health care emphasizes the importance of nurses understanding the theoretical concerns associated with such approaches. When evaluating questionnaire results, the development process should be described in sufficient detail and rigor to allow a practitioner to make an informed decision on whether to incorporate the findings. We use questionnaires to collect information in a structured manner that, when collected from a representative sample of a given population, helps us to extrapolate findings to the larger population. This is critical when assessing the efficacy of care or treatment. Although the rigor with which questionnaires are developed is improving, nurses are still not necessarily adept or secure in the psychometric assessment of such tests (Jones & Johnston 1999). The issues of reliability and validity that underpin questionnaire production from item generation to the proposal of an a priori factor structure to subsequent psychometric analysis are central to understanding the findings obtained from questionnaires. Although applicable texts which contain details about these issues, there is rarely enough detail in a single source to direct the questionnaire creation process. This write-up offers a critical examination of key methodological problems ranging from item generation to expected psychometric assessment, as well as a collection of heuristic decision-making tools to help practitioners create their own measures or assess the work of others. Two case studies from clinical practice and nurse education demonstrate these. Item analysis and exploratory factor analytic approaches are used to investigate issues of reliability and validity.
