In May 1968, the Chief of Army Staff, Lieutenant Colonel Joseph Akahan, aged 31 died in a helicopter crash.

On October 15, 1969, the then Chief of Air Staff, Colonel Shittu Akanji Alao seconded from the Army died on active duty while on a solo flight in an L-29 aircraft in Uzebba, a town near Benin city. He ran into bad weather and subsequently ran out of fuel. He tried to make an emergency landing, but instead, ran into a tree and died in the process. He was 32 years old.

There was nothing like Chief of army staff(coas) in 1968. What was obtainable was Chief of Staff,Army(Cosa). The last person that occupied that rank before it was changed to Chief of army staff(coas) was Lt,gen.Ejoor. I think he died few years back in Delta ( he was also the military governor of the mid-western region back then).
