Home health care for people with dementia comes at a large cost once the person’s dementia has deteriorated. In many cases within Adult Social Care, they are in need of home health care support to remember to do daily tasks and also support in carrying out complex and regular responsibilities.

Emotionally, dementia is a drain on both the person and their family. Adult Social Care has used our home health care service within the early stages of dementia to allow their loved ones to continue living independently, whether that be to remind them to take vital medication that in the long term prevents their dementia from worsening or simply to remind them to carry out simple tasks such as eating lunch, walking pets, attending day centers or appointments and more.

This has long-term benefits also as it can reduce hospital admissions, doctor surgery appointments and even delay/prevent the need to move to a care home. Below are specific examples of how we can help from Adult Social Care workers:

Reminded & Educated to eat Lunch every day
Mrs. B was referred to the Reablement team and it was assessed that she was forgetting to eat lunch every day and therefore was becoming malnourished.

Despite the previous routine would be to commission meals on wheels or some form of home health care such as a carer to visit each lunchtime, the council were supportive in using alternative methods and decided to test a personalized message by the daughter to remind and educate Mrs. C to prepare and eat her lunch.

Having recorded various different messages, Mrs. C received calls to remind her to eat her lunch and also what and how to prepare. When instructed to eat and what to prepare, a home health care visit each week confirmed that there was evidence to prove that she was eating every lunchtime now. A further visit to the GPs confirmed that her health had improved too.

After Traumatic Event, Hospital Discharge sets up service
Mr C was recently admitted to the hospital and has since obtained a medication regimen that he is completely new to. The medication is important to his recovery so as a precaution, the Adult Social Care team contacted us to set up a Medication Reminder service for Mr B.

For the next 12 weeks he received calls three times per day to remind him to take the medication. On review, it was agreed that he had successfully taken all of his medication at the correct times. Mr B chose to continue with the service as a safety net and to ensure his family that he was ok.

Mild Dementia doesn’t stop lady from being independent
Mrs D’s daughter was worried that Mrs D would soon need more advanced home health care and wanted to trial a service that would maintain her independence and keep her in her own home as she would prefer.

Mrs D’s daughter set up four personalized messages that would contact her Mother to remind her to carry daily tasks. A 9am call would ask Mum to take her medication, a 1pm call would remind her that its time to eat, and then to calls in the evening would suggest her Mother that she should let the dog out before she goes to bed, and remember to let the dog back in.
