Answering such questions has to be by real life experience that the individual had in missing a deadline while working.
However, missed deadlines can be as a result of a number of factors. You can coin your answers around some suggestions which I have tried to enumerate some below.
1. The scope of the work was underestimated, hence, the time of completion of the work was not properly estimated.
2. Deadline can be missed if someone elses work feeds into yours and the person did not complete his part on time.
3. Can be due to additional responsibilities that were given to you which were not considered when you took on the first assignment to deliver.
4. Inadequate information required to complete the assignment, which you had to source for, as a result time was lost on the job.
5. Lack of adequate tools to perform the work, resulting in improvising or requesting for the tools before continuing the job.
6. Not performing work as efficiently as required (you can't say this in an interview I suppose).

Repercussions that you can mention in an interview?
Difficult to say. But here is what missing deadlines usually result in:

1. Financial loss on the part of the company.
2. Re-allocation of the work assignment to someone else, hence negative record on your part.
3. A company can loose a client or a job as a result of you missing deadline for a proposal (for instance).
