Some sex scenes make use of a body double like when stunts are done by stunts men/women. This is usually required if the main actor/actress is not in agreement with the act required.

There are some unprofessional actors that take advantage of the scene to actually engage in intercourse with their movie partners despite it being off script. I've see a Nigerian movie making where an actress slapped the guy when he actually let it slip in.
