Dear Sir/Ma,

RE : MRS BERYL CASEY/F/DOB 21/11/1941

I am writing to you regarding Mrs Beryl Casey who recently had a Left Hemiarthroplasty. She requires further assessment for Rehabilitation Therapy. (1.5 lines)

Mrs Casey had a fainting spell which led to a fall as she made attempts to get up from bed. X-ray done confirmed Fractured left neck of femur. A surgical procedure has been done to fix the fracture, significant progress has been made afterwards. (2.5 lines)

She is a known hypertensive maintained on Tab Ramipril 5mg daily, also on Tab Panadol 1g qid as necessary. A repeat Hemoglobin level test would be needed, in addition to BP checks twice daily for 3 days. Currently, Mrs Casey wears compression stockings and needs support for daily activities. (3 lines).

Her family should be informed about her transfer from the hospital which is already scheduled for 11am. An appointment for staple removal would be necessary in the next few days. (1.5 lines)
