The form is still on. You could pick it up in any branch of Intercontinental Bank for 2k only!

I'm not very sure but i think the PTDF scholarship form collection and submission are ova.
If u still want to try,pls go to d state scholarship board nearest to you,u would then purchase a
bank draft at the neartest 1stbank not intercontinental bank.this will b used to collect d form
4m the scholarship board. there is no harm in trying,let my 1st line not discourage u. gudluck

If u re still interested in the PTDF Oversea Scholarship form,and u reside in ABJ,u can get the form in the:

DEPT OF POLICY & IMPLEMENTATION
EDUCATION SECRETARIAT (FCDA)
PLOT 137 JABI
FCT ABUJA.

If u stay outisde ABJ u can breeze in and get the form b4 the deadline.
