To be frank the title Doctor was initially meant for PhD holders before MBBS smuggled the title, they were called Physicians. He is a licensed professional qualified to treat patients through physical methods like electrotherapy, manipulative therapy, exercise therapy etc. The difference between the physician and the physiotherapist is treatment with drugs.
