The only additional document you will require is your police report and court affidavit.

You can get these from the police station and courts.

Also go along with 4 passport photographs.

Note: This is for people whose information will remain the same I.e. the information in your lost/damaged passport has not changed, and you want to get a replacement.

My date of birth was wrongly inputted on the slip. How can I correct it

You will have to pay N15,000 on the remita platform, then take your payment receipt and the NIN slip to NIMC office, before it can be changed.
This is the only modification that's unnecessarily expensive. Follow the steps in the video and make payment online.
