Family planning methods are not a 100% effective...... many are just >90%.

My opinion

Medical science is conducted and practiced by human. There is likelihood of error to some varying degree, since humans are involved. Even sometimes, the reaction from one's body system can alter the programmed setting in the body.

It also tells you that science also has limitations. It happens at times.

Some women deliberately do stuff like this, it's not like the family planning method failed.

It depends on the type of family planning used.
Most of them are not 100% safe..

Some of the sure way of preventing pregnancies are vasectomy(cutting the tubes that carries sperm cells from the testes to the penis) in males, cutting of the falopian tubes in females, IUD(intrauterine device implantation in females) etc though some have side effects on the long run.

Others like use of condoms(male&female), contraceptive pills and creams either before or after sexual acts, avoidance of sex during ovulation periods and so on are still prone to errors and mistakes.

The surest way of preventing pregnancies with no worries is complete abstinence.

I think it would have been more appropriate if the heading went like ....."get pregnant despite family planning".
As to your question, there are a few factors which could be considered, chief of which is human factors.
Most contraceptives have efficacy of 98-99% when used correctly, meaning than 1 or 2 in a 100 women would get pregnant in 1 year despite appropriate use of contraceptives.
The question is: are women (and men) using them correctly?
Do men know how to properly wear a condom? (Don't laugh o)
Do women always remember to take their pills?
Do women take drugs or other substances that could make the contraceptive less effective?
Many people don't know that there is a long list of drugs that could reduce the effectiveness of contraceptives, never mind agbo. And our doctors or chemists fail to warn about these.

The best is to use condom.

It's as simple as these procedures are not fool-proof.
