Success mail here but I didn't get the attach document we're supposed to fill. Any ideas?

Same here, I am considering replying the mail, notifying them that there is not form attached to my mail.

Update tto those that did not get the PHR forms attachment, I sent a mail and I have been replied with the PHR attachment.
