What's ur LGA of residence.........? That's where u should go 4 ur physical verification, ur data will not be found in any other LGA. It happend to me until i find the right LGA 4 my physical verification.

Yes, you are to attach one passport photograph and photocopies of some documents in a file and submit

Ma LGA of residence was where I went first and ma data was not found there.
