Let me break it down to simpler wording.
Black communities are usually poorer,
which means more poorer diets (which leads to obesity and diabetes),
and avoiding hospitals (can't afford the bills or healthcare coverage) except for server sickness or injuries, which means poorer cancer screenings.

Black communities are much poorer, because... drum roll... racism. Being redlined, being left out of the GI bill post ww2, and not being allowed to build wealth. So being called the Nword directly and indirectly.
