Yes, I was allowed to submit. Every other document has my middle name, only the application form, which was even returned to me at TLS.

What error did you make?

The error was made with my french visa form...I used the copy functionality but forgot to change the passport and so the two forms had the same passport number, date of issue and expiry. All other details are correct. This mistake was only done in the french Visa Form and not in TLS Contact form.

I sincerely have no idea if this would fall into the same category as mine. Mine was omission of my middle name from the French visa form.

I will defer to experts in the house to weigh-in on this....

It's Ok to print only the first page. I printed the first 3 pages, but the TLS official took only the first page and returned the rest to me.

This is AXA, the French insurance giant. You’re insuring directly with them.
Their Nigerian subsidiary is called AXA Mansard, and is a member of the AXA group.
That’s the one most people uses. Both should be valid. https://corporate.axamansard.com/our-profile/

In your case, did you later generate another french visa form, and submit both the wrong and the correct one just as their FAQ suggests?

Please make Una help me.

I filled my application form and put my travel date for September which I haven't submitted the form yet. I also filled the same in my appointment form but the appointment is now on October 22nd which I have booked hoping to make the necessary changes of my travel dates in my application form.

Now the issue is, I forgot I also need to correct the travel date in my appointment form before booking for an appointment which I didn't correct.

Pls Is there a solution?

Yes, I generated another French application form, but it carries another applicant ID. Nonetheless, I took it along to my appointment but only presented the first one that carries the same ID as my TLS appointment confirmation. I added the 2nd one to my supporting documents. The TLS agent returned both the first and the corrected forms to me. She printed some forms, added them to my application envelope and gave some to me - all of them had my middle name on it.

When I got home after the appointment, I logged on to my profile and saw that the original application form now has my middle name on it.

Nice, but if you call tls customer service. they'll ask you to pay for another booking... After submission do you receive any other confirmation? Like been received at the consulate or sent to the consulate
